Below is a list of Tournament specific forms for teams and players eneterd into District 22 Majors, 9/10 All-Stars, 10/11 All-Stars and 11/12 All-Stars. Each of the forms listed below may be filled out electronically, then printed for signatures.Please complete all required forms and return to your manager by the date designated in your email. manager must contact the league President for Distcrict 22 review and approval of the team to particpate in the tournamants.
